Heartworm Prevention Medications

Several safe and effective heartworm preventive medications are available by prescription through veterinarians. The most common active ingredients are macrocyclic lactones (ivermectin, milbemycin, selamectin, moxidectin), which work by eliminating the infective larvae deposited by mosquitoes before they can mature into adult worms. These preventives are generally given on a monthly schedule and are formulated as chewable tablets, topical spot-on treatments, or injectable products.

Monthly oral products include Heartgard (ivermectin), Interceptor (milbemycin oxime), Sentinel (milbemycin plus lufenuron for flea larvae control), and Trifexis (milbemycin plus spinosad for fleas). Topical options include Revolution (selamectin), which also controls fleas, ear mites, and certain ticks. ProHeart 6 and ProHeart 12 are injectable moxidectin products administered by a veterinarian every six or twelve months, respectively, eliminating the need for monthly owner-administered doses and removing compliance as a variable.

Because the test detects adult worm antigen rather than the preventive drug itself, it functions as a check on whether any infection has occurred despite prevention and serves as a baseline before starting a puppy on preventives.

Testing before starting preventives is essential because giving a heartworm preventive to a dog with an existing adult heartworm infection can cause a severe and potentially fatal reaction if large numbers of microfilariae are present. A dog with an undetected infection that receives a monthly preventive may experience a sudden die-off of microfilariae that triggers a systemic inflammatory reaction. For this reason, dogs whose preventive history is unknown or inconsistent should be tested before starting or resuming prevention.

The cost of year-round prevention is significantly lower than the cost of heartworm treatment, which typically ranges from $1,000 to $2,000 or more depending on the severity of infection. Monthly preventives generally cost between $6 and $18 per month depending on the dog’s weight and the specific product, making consistent prevention both medically and financially sensible.
