What Is Feline Leukemia Virus?

Feline leukemia virus (FeLV) is one of the most serious infectious diseases in domestic cats. It is a retrovirus that inserts its genetic material into the host cat’s DNA, making elimination extremely difficult. FeLV primarily attacks the immune system, leaving infected cats vulnerable to secondary infections and cancers. First identified in the 1960s, FeLV remains a top cause of cat mortality in multi-cat households.

Unlike many viral diseases, FeLV does not always cause immediate illness. Some cats mount a strong immune response and eliminate the infection (regressive infection), while others develop persistent viremia leading to progressive disease. Understanding which cats are most at risk helps owners make informed decisions about vaccination, testing, and care.

How FeLV Spreads Between Cats

FeLV is shed in saliva, nasal secretions, urine, feces, and breast milk. Transmission requires prolonged, close contact — grooming, sharing food bowls, and sharing litter boxes all pose significant risk. The virus does not survive long outside the host (a few hours), so casual brief contact is far less dangerous than sustained exposure.

Kittens and young cats are most susceptible because their immune systems are still maturing. A queen with FeLV can pass the virus to her kittens in utero or through nursing. Adult cats develop stronger natural resistance over time, but no cat is completely immune without vaccination. Multi-cat households and outdoor cat colonies carry the highest transmission risk. Importantly, FeLV is species-specific and cannot infect dogs, people, or other animals.

Symptoms and Disease Progression

In early stages, some cats show mild flu-like signs — lethargy, mild fever, swollen lymph nodes — before the virus either clears or establishes a permanent infection. Cats in the progressive stage may appear completely healthy for months or years before serious disease develops.

When FeLV-associated disease emerges, it commonly takes the form of lymphoma (FeLV-positive cats develop lymphoma at rates many times higher than uninfected cats) or anemia. Owners may notice pale gums, extreme lethargy, rapid breathing, and poor appetite — warning signs of significant red blood cell loss. Secondary infections occur because FeLV impairs T-cell and B-cell function, leading to chronic respiratory, skin, and digestive problems that would be uncommon in a healthy cat.

Diagnosis, Treatment, and Management

Diagnosis uses an ELISA test that detects FeLV proteins in the blood — available at most veterinary clinics and gives rapid results. A positive ELISA should be confirmed with an IFA test or PCR to distinguish regressive from progressive infection.

There is no cure for FeLV, but management focuses on quality of life and treating secondary conditions. Antiviral medications such as interferon omega have shown modest benefit. Prompt treatment of infections, high-quality nutrition, stress minimization, and regular veterinary checkups (every six months) can significantly extend a cat’s comfortable life. FeLV-positive cats should be kept strictly indoors to protect themselves and prevent spreading the virus to other cats.

Vaccination and Prevention Strategies

The FeLV vaccine is a non-core vaccine recommended based on individual risk factors. For kittens, outdoor cats, and cats in multi-cat households where FeLV status is unknown, vaccination is strongly advised. The AAFP recommends vaccinating all kittens, then reassessing risk in adulthood.

The vaccine dramatically reduces the risk of progressive infection. It is most effective when given before any virus exposure. Vaccination combined with testing all new cats before introducing them to a household, maintaining indoor-only lifestyles, and using separate food bowls and litter boxes in multi-cat homes forms a comprehensive prevention strategy. Shelters routinely test cats for FeLV before adoption. Breeders should test breeding stock to prevent vertical transmission to kittens.
