Treatment: Surgery vs. Medical Management

Surgical options (best long-term outcomes):

  • Triple Pelvic Osteotomy (TPO) — best for young dogs under 10 months with no arthritis; pelvis is cut and repositioned to improve socket coverage
  • Juvenile Pubic Symphysiodesis (JPS) — simple procedure before 20 weeks that fuses part of the pelvis to improve hip angles
  • Femoral Head and Neck Excision (FHO) — removes the ball, allowing a “false joint” to form; most effective in smaller dogs under 20 kg
  • Total Hip Replacement (THR) — gold standard for severe cases in skeletally mature dogs; often returns dogs to near-normal function

Medical management (mild-moderate cases or dogs unsuited to surgery):

  • Weight management — the single most effective non-surgical intervention
  • Controlled low-impact exercise — swimming and leash walks rather than jumping or fetching
  • NSAIDs (carprofen, meloxicam) for pain and inflammation
  • Joint supplements (glucosamine, chondroitin, omega-3s, green-lipped mussel)
  • Physical rehabilitation and hydrotherapy
  • Adequan injections to support cartilage health
  • Laser therapy and acupuncture as adjuncts

Living With a Dog That Has Hip Dysplasia

With appropriate care, many affected dogs live happy, comfortable lives. Key management strategies include: maintaining a lean body weight, providing orthopaedic bedding, using ramps instead of stairs, maintaining regular gentle exercise, scheduling regular vet check-ins as arthritis progresses, and considering certified canine rehabilitation therapy to strengthen supporting muscles.
