Feline Pancreatitis: Symptoms, Diagnosis & Treatment 2026

Pancreatitis in cats is one of the most frequently underdiagnosed conditions in feline medicine — partly because cats are notoriously subtle in how they display pain and illness, and partly because the symptoms overlap with so many other conditions. Yet pancreatitis is common, often serious, and closely linked to other major feline diseases. Understanding it can make the difference between early intervention and a missed diagnosis.

What Is Feline Pancreatitis?

The pancreas is a gland located near the stomach and small intestine that has two major functions: producing digestive enzymes (exocrine function) and producing hormones including insulin and glucagon (endocrine function). Pancreatitis occurs when the pancreas becomes inflamed — often because digestive enzymes that should be inactive until reaching the small intestine become prematurely activated inside the pancreas itself, causing the organ to essentially begin digesting itself.

Feline pancreatitis is classified as acute (sudden onset, potentially severe) or chronic (ongoing, lower-grade inflammation that persists over time). Chronic pancreatitis is more common in cats than in dogs, and many cats live with low-level chronic pancreatitis that flares periodically. The distinction matters because treatment approaches and prognosis differ.

An important feature of feline pancreatitis is its association with two other common feline diseases: inflammatory bowel disease (IBD) and cholangitis/cholangiohepatitis (inflammation of the bile ducts and liver). The combination of all three is so common in cats it has been given the name “triaditis.” Cats presenting with pancreatitis should routinely be evaluated for these concurrent conditions.

Causes of Pancreatitis in Cats

In most cats, the cause of pancreatitis is idiopathic — meaning no specific underlying cause can be identified. This is in contrast to pancreatitis in humans and dogs, where dietary indiscretion, high-fat meals, and certain drugs are more commonly implicated triggers.

Identified or suspected causes in cats include infectious diseases — particularly toxoplasmosis and feline infectious peritonitis (FIP), and possibly feline herpesvirus. Trauma to the abdomen can trigger pancreatitis. Certain medications, including some antibiotics, chemotherapy drugs, and organophosphate insecticides, have been implicated. Parasites, particularly liver flukes, can cause inflammation that spreads to the pancreas. Hypercalcemia (high blood calcium levels) is associated with pancreatitis in some cats.

Symptoms of Feline Pancreatitis

One of the reasons pancreatitis is so commonly missed in cats is that their symptoms are vague, non-specific, and often subtle. Unlike dogs with pancreatitis — which classically present with vomiting, abdominal pain, and a hunched posture — cats frequently show far more ambiguous signs.

The most common signs in cats include loss of appetite (anorexia), which may range from reduced interest in food to complete refusal to eat. Lethargy and weakness — the cat seems quiet, withdrawn, and less active than normal — is nearly universal. Weight loss is common, particularly in chronic pancreatitis. Dehydration is frequently present. Fever occurs in some cats but not all. Vomiting occurs in a minority of cats with pancreatitis — far less commonly than in dogs — and abdominal pain, while present, is often not overtly displayed by cats, who mask discomfort as a survival instinct.

In severe acute cases, cats may show jaundice (yellowing of the skin, whites of the eyes, or gums) if liver involvement is present, profound weakness, and signs of systemic illness. Occasionally, cats with severe acute pancreatitis develop disseminated intravascular coagulation (DIC) — a life-threatening clotting disorder — or multi-organ failure.

Diagnosis of Feline Pancreatitis

Diagnosing pancreatitis in cats requires a combination of tests, as no single test is definitive.

The most useful blood test is the feline pancreatic lipase immunoreactivity (fPLI) test, which is highly specific to the feline pancreas and the most sensitive blood marker available. A high result strongly supports pancreatitis; however, mildly elevated values need to be interpreted alongside clinical signs. Standard blood panels (complete blood count, biochemistry) and urinalysis are used to assess organ function and look for concurrent disease. Abdominal ultrasound is the most useful imaging tool — an experienced ultrasonographer can identify changes in pancreatic echogenicity, size, and surrounding tissue. In severe cases, the pancreas may have a “ground-glass” appearance with surrounding peritoneal effusion.

Treatment and Management

Treatment for feline pancreatitis is primarily supportive, as there is no specific anti-pancreatitis drug available. Hospitalization is often needed for moderate to severe cases.

Intravenous fluid therapy corrects dehydration and supports organ perfusion. Pain management is critical — cats experience significant pain with pancreatitis and need appropriate analgesic medications. Anti-nausea medications (like maropitant or ondansetron) are used when vomiting or nausea is present. Nutritional support is a key priority: unlike the old “rest the pancreas” approach of withholding food, current evidence strongly supports early enteral feeding in cats with pancreatitis, as prolonged fasting increases the risk of hepatic lipidosis (fatty liver disease). If the cat won’t eat voluntarily, a feeding tube may be placed.

Concurrent conditions — IBD, cholangitis, infections — are treated simultaneously. Long-term management of chronic pancreatitis may include a low-fat, easily digestible diet, management of concurrent diseases, and cobalamin (vitamin B12) supplementation, as pancreatitis often impairs B12 absorption.
