Canine distemper is a highly contagious and potentially fatal viral disease that affects dogs worldwide. Despite the availability of an effective vaccine, it remains one of the most serious infectious threats to unvaccinated dogs and certain wildlife populations. Understanding what distemper is, how it spreads, and how to protect your dog is essential for every pet owner.

What Is Canine Distemper?

Canine distemper is caused by the canine distemper virus (CDV), a paramyxovirus closely related to the measles virus in humans. It was one of the leading causes of dog death before vaccination became widespread in the mid-20th century. The virus attacks the respiratory, gastrointestinal, and nervous systems, making it a multi-systemic disease with a broad and often devastating range of effects.

CDV infects not only domestic dogs but also ferrets, raccoons, foxes, wolves, coyotes, mink, skunks, and some large wild cats. Outbreaks in wildlife populations — particularly raccoons — can serve as a reservoir that exposes unvaccinated domestic dogs. There is no cure for distemper; treatment is supportive and focused on managing symptoms.

Mortality rates for distemper vary depending on the dog’s age, immune status, and which body systems are affected. Young puppies, senior dogs, and immunocompromised animals face the highest risk of death. Dogs that survive the acute phase may still develop permanent neurological damage.

How Canine Distemper Spreads

Distemper spreads primarily through airborne exposure — respiratory droplets released when an infected animal coughs, sneezes, or barks. The virus can also be transmitted through direct contact with infected bodily fluids including saliva, nasal discharge, urine, and feces. Sharing food and water bowls with an infected animal is a transmission route, as is contact with contaminated surfaces.

An infected dog can begin shedding the virus before showing visible symptoms, making it difficult to identify and quarantine infectious animals early. The incubation period is typically 3 to 6 days after exposure, though neurological signs can appear weeks or even months after the initial infection.

Settings with high dog density — shelters, dog shows, pet stores, and boarding facilities — pose elevated exposure risks for unvaccinated animals. Puppies are particularly vulnerable because their immune systems are still developing and maternal antibodies begin to wane at 6 to 8 weeks of age.

Signs and Symptoms of Canine Distemper

Distemper presents in three overlapping phases, affecting different body systems at different stages of illness.

The respiratory phase typically appears first. Symptoms include nasal and ocular discharge (starting watery and becoming thick and purulent), coughing, sneezing, labored breathing, and fever. Dogs may appear lethargic and lose their appetite. This phase can look similar to kennel cough or canine influenza.

The gastrointestinal phase follows or overlaps, bringing vomiting, diarrhea (sometimes bloody), and dehydration. Weight loss and weakness become pronounced. A characteristic sign of advanced distemper is hyperkeratosis — the hardening and thickening of the foot pads and nose, sometimes called “hard pad disease.”

The most severe stage involves neurological signs. These may include muscle twitching (particularly of the jaw, limbs, or whole body), seizures, circling, head tilt, incoordination, paralysis, and behavioral changes. Some dogs develop “chewing gum fits” — rhythmic jaw movements resembling chewing. Neurological symptoms can appear during acute illness or weeks to months later in dogs that appeared to have recovered.

Diagnosis and Treatment

There is no single definitive test for distemper. Diagnosis is based on clinical signs, vaccination history, exposure risk, and laboratory results. Tests may include blood counts (looking for low lymphocyte counts), RT-PCR testing of nasal swabs or cerebrospinal fluid, and specialized staining of conjunctival or respiratory cells to look for viral inclusions.

Since no antiviral drug exists for CDV, treatment is entirely supportive. Goals include preventing secondary bacterial infections (using antibiotics), managing fever, providing IV fluids for dehydration, controlling seizures with anticonvulsant medication, ensuring adequate nutrition, and keeping the dog comfortable. Infected dogs must be strictly isolated from other animals to prevent spread.

Prognosis depends heavily on which body systems are involved. Dogs with primarily respiratory and GI signs have a better chance of recovery than those with neurological involvement. Neurological damage, if severe, often does not reverse even after the virus is cleared, and many dogs with significant neuro signs are humanely euthanized.

Vaccination: The Best Protection Against Distemper

The distemper vaccine is one of the core vaccines recommended for all dogs by the American Animal Hospital Association (AAHA). It is given as part of the DA2PP or DHPP combination vaccine, which also covers adenovirus, parvovirus, and parainfluenza. This vaccine is highly effective and has dramatically reduced the incidence of distemper in vaccinated populations.

Puppies receive a series of vaccines starting at 6 to 8 weeks of age, with boosters every 3 to 4 weeks until 16 weeks of age. Adult dogs with an unknown vaccination history receive a booster, then another a year later. After the initial series, boosters are given every 1 to 3 years depending on the product used and your vet’s recommendation. Keeping up with this schedule is the single most important step you can take to protect your dog from distemper.
