Wobbler syndrome is a serious neurological condition that causes a characteristic wobbling or unsteady gait in affected dogs. What Is Wobbler Syndrome?

Wobbler syndrome — formally called cervical spondylomyelopathy (CSM) — is a disease of the cervical (neck) spine causing compression of the spinal cord and/or nerve roots in the neck region. Signs and Symptoms

MRI is the gold standard — it provides detailed images of the spinal cord, discs, and ligaments and is essential for surgical planning. CT myelogram (CT with contrast injected into the spinal canal) is used when MRI is unavailable. Other conditions mimicking wobbler syndrome — degenerative myelopathy, peripheral neuropathies, and other spinal cord diseases — must be excluded through appropriate testing.

Treatment Options

Conservative (medical) management is appropriate for mildly affected dogs or those at high surgical risk. It involves: strict exercise restriction (leash walks only, no jumping) for 6–8 weeks; anti-inflammatory medications; pain management; physiotherapy; and using a harness instead of a neck collar. Success rates for mild cases are 50–60%, though recurrence is common. Surgical decompression is recommended for moderate to severe signs, worsening progression, or failure of conservative management. Multiple techniques exist — ventral slot, dorsal laminectomy, and distraction-fusion — chosen based on the type and location of compression on MRI. Approximately 80% of appropriate surgical candidates show improvement. Recovery requires strict rest, physiotherapy, and hydrotherapy to support neurological recovery.

Prognosis and Life With Wobbler Syndrome

Prognosis depends heavily on severity at the time of treatment. Dogs treated early (mild signs) have the best outcomes. Dogs still ambulatory at surgery time have significantly better prognoses than those already paralyzed. Full neurological recovery is not guaranteed but is possible, particularly with early intervention. Lifelong management modifications are necessary: harness instead of collar, avoiding stairs and jumping, maintaining healthy weight, and regular monitoring with a neurologist.
