What Is Cat Diabetes?

Feline diabetes mellitus is a chronic metabolic disease in which a cat’s body fails to properly regulate blood glucose (blood sugar). It occurs when the pancreas doesn’t produce enough insulin, or when the body’s cells don’t respond adequately to the insulin produced.

Feline diabetes most closely resembles human Type 2 diabetes — insulin resistance is the predominant mechanism, often driven by obesity and a high-carbohydrate diet. Unlike dogs, a meaningful proportion of cats — approximately 20–40% — can achieve diabetic remission with early, aggressive treatment. This makes early diagnosis and prompt management especially important in cats.

Diabetes affects approximately 1 in 230 cats. Overweight male cats over 8 years of age are at highest risk, though any cat can develop diabetes. Certain breeds, including Burmese cats, have a known genetic predisposition.

Risk Factors for Cat Diabetes

Several factors significantly increase a cat’s likelihood of developing diabetes:

  • Obesity — by far the biggest risk factor; overweight cats are 4× more likely to develop diabetes
  • High-carbohydrate diet — dry food diets promote persistent blood glucose spikes in obligate carnivores
  • Male sex — male cats are affected roughly twice as often as females
  • Age — risk increases substantially after age 8
  • Neutered status — neutered cats have higher obesity rates, indirectly increasing risk
  • Long-term steroid use — corticosteroids for conditions like allergies can induce diabetes
  • Chronic pancreatitis
  • Acromegaly — excess growth hormone (GH) drives severe insulin resistance; underdiagnosed in cats

Symptoms of Diabetes in Cats

The classic symptoms of feline diabetes mirror those seen in other diabetic species:

  • Increased thirst (polydipsia) — cats drinking noticeably more water
  • Increased urination (polyuria) — larger urine clumps in litter box, accidents outside the box
  • Increased appetite (polyphagia) — seeming ravenously hungry despite eating
  • Weight loss — despite eating well, the body catabolizes fat and muscle for energy
  • Plantigrade stance — cats walking flat-footed on the full heel rather than their toes; a sign of diabetic neuropathy affecting the hind limbs
  • Poor, unkempt coat condition
  • Lethargy and weakness
  • Vomiting (especially with diabetic ketoacidosis)

The plantigrade stance (hocks touching the ground when walking) is a distinctive sign of feline diabetes that many owners miss. It can improve significantly with proper glucose regulation. Diabetic ketoacidosis (DKA) is a medical emergency in cats — watch for collapse, fruity breath, and profound weakness.

Diagnosis and Treatment of Cat Diabetes

Diagnosis requires demonstrating persistent hyperglycemia and glucosuria (glucose in urine). An important complication in cats: stress hyperglycemia — cats can have transiently elevated blood glucose from the stress of a vet visit alone, which can mimic diabetes. Your vet may recommend a fructosamine test — a marker that reflects average blood glucose over the past 2–3 weeks — to distinguish true diabetes from stress-related glucose spikes.

Insulin therapy is the standard of care. The most commonly used insulins in cats are:

  • ProZinc (protamine zinc insulin) — FDA-approved for cats, long-acting, given once or twice daily
  • Glargine (Lantus) — a human long-acting insulin widely used off-label in cats and strongly associated with remission
  • Detemir (Levemir) — another long-acting human insulin used in cats

Alongside insulin, dietary change is crucial and can dramatically improve regulation. The current evidence strongly favors a low-carbohydrate, high-protein diet — essentially canned/wet food with less than 10% carbohydrates on a dry matter basis. This reduces the glucose load from meals, making insulin requirements lower and remission more achievable.

Home blood glucose monitoring with a veterinary or human glucometer (using ear vein or paw pad) is standard practice for motivated owners and is key to achieving remission.

Can Diabetic Cats Go Into Remission?

Yes — and this is one of the most encouraging aspects of feline diabetes management. Diabetic remission means the cat no longer requires insulin to maintain normal blood glucose levels. Studies suggest remission rates of 20–40% or higher with the right approach, rising to over 50–60% in some studies using glargine or detemir insulin combined with a low-carbohydrate diet.

Factors that improve chances of remission:

  • Early diagnosis and aggressive treatment before beta cell burnout
  • Use of long-acting insulin (glargine or detemir) targeting near-normal glucose
  • Low-carbohydrate, high-protein diet
  • Weight loss if the cat is obese
  • Elimination of any concurrent conditions (e.g., acromegaly, which greatly reduces remission chances)

Even cats that don’t achieve full remission can live comfortably for many years with consistent management. Regular monitoring is essential to catch hypoglycemia (especially as cats approach remission and need less insulin).
