What Is Dog Vestibular Disease?
The vestibular system is responsible for your dog’s sense of balance and spatial orientation. It includes the inner ear and parts of the brainstem that coordinate eye movement, posture, and equilibrium. When this system is disrupted — whether by infection, injury, or unknown causes — it results in what veterinarians call vestibular syndrome.
There are two main types: peripheral vestibular disease (originating in the inner or middle ear) and central vestibular disease (originating in the brainstem or cerebellum). Peripheral vestibular disease — also called idiopathic vestibular disease or “old dog vestibular syndrome” — is by far the most common and generally the least serious.
Idiopathic vestibular disease accounts for the majority of cases and is most common in older dogs, often appearing suddenly overnight with no warning signs. The name “idiopathic” simply means the exact cause is unknown.
Symptoms to Watch For
The symptoms of vestibular disease are dramatic and often alarming. Many owners mistake the episode for a stroke or seizure. Signs typically appear suddenly and may include:
- Head tilt — often to one side, sometimes severe
- Loss of balance — stumbling, falling, or rolling
- Nystagmus — rapid, involuntary eye movements (horizontal, vertical, or rotary)
- Circling — walking in circles toward the affected side
- Vomiting or nausea — from the dizziness
- Reluctance to move — due to disorientation
- Wide-legged stance — attempting to stay upright
Importantly, most dogs with idiopathic vestibular disease remain alert and conscious — they know you’re there, they just can’t balance. This is a key difference from a stroke. Facial paralysis may also occur in cases involving the inner ear.

Common Causes
The cause varies depending on whether vestibular disease is peripheral or central:
Peripheral causes include idiopathic (unknown — most common in senior dogs), inner or middle ear infections (otitis interna/media), ear polyps or tumors, hypothyroidism, certain medications (particularly aminoglycoside antibiotics like gentamicin), and trauma to the ear. Idiopathic vestibular disease is most common in dogs over 8 years old and shows no seasonal pattern — it can strike any breed at any time.
Central causes include brain tumors, inflammatory diseases like granulomatous meningoencephalitis (GME), strokes (cerebrovascular accidents), infections such as distemper, and metabolic disorders affecting the brain. Central vestibular disease is more serious and requires urgent veterinary evaluation.
Certain breeds may have a higher predisposition to ear problems that trigger vestibular episodes, including Cocker Spaniels, Beagles, and Bulldogs — breeds with anatomy that makes them prone to ear infections. However, idiopathic disease can affect any breed.
Diagnosis & Treatment
There is no single definitive test for idiopathic vestibular disease. Instead, veterinarians make a diagnosis of exclusion — ruling out other causes through a combination of history, physical exam, and diagnostics.
Your vet may perform an otoscopic exam (checking the ear canal and eardrum), blood and urine tests to rule out thyroid disease or systemic illness, and in some cases, advanced imaging such as MRI or CT scan — especially if central vestibular disease is suspected. Referral to a veterinary neurologist may be recommended if symptoms don’t improve or if central disease is possible.

For idiopathic vestibular disease, treatment is largely supportive. Anti-nausea medication (such as meclizine) can help with the vomiting and dizziness. Keeping your dog safe, comfortable, and well-hydrated is the primary goal. Most dogs begin improving within 72 hours.
Recovery Timeline & Home Care
The recovery from idiopathic vestibular disease is generally excellent. Most dogs show noticeable improvement within 48–72 hours, and the majority return to near-normal function within 2–4 weeks. Some dogs may retain a permanent mild head tilt, but this does not affect their quality of life.
During recovery, you can support your dog at home by keeping them in a safe, confined space to prevent falls and injury. Block off stairs, keep food and water easily accessible, and help them outside for toilet breaks. If nausea is severe and your dog won’t eat or drink, contact your vet about anti-nausea medication or subcutaneous fluids.
You may notice the head tilt persists even after balance improves — this is normal and usually fades over weeks to months. If your dog is not improving by day 3 or is getting worse, revisit your vet, as this could indicate central vestibular disease or another underlying condition requiring urgent intervention.
